By submitting this electronic application, I hereby apply for electric service and agree to take such service from Pascoag Utility District in accordance with the Terms and Conditions on file with the Rhode Island Public Utilities Commission. If my account becomes delinquent, I agree to pay all costs of collection, including all reasonable attorneys’ fees and costs. Once I submit this application, I will receive an email with a PDF copy of Pascoag Utility District’s Electric Terms and Conditions. I further authorize Pascoag Utility District (“Pascoag” or “District”) to contact my employer to verify my employment and verify any documentation that is delivered to the District.

I hereby apply for water service and agree to take such service in accordance with the Pascoag Utility District rules, term and conditions, and schedule of rates governing this class of service. If my account becomes delinquent, I agree to pay all costs of collection, including all reasonable attorneys’ fees and costs. Once I submit this application, I will receive an email with a PDF copy of Pascoag Utility District’s Water Rules and Regulations. I further authorize Pascoag Utility District (“Pascoag” or “District”) to contact my employer to verify my employment and verify any documentation that is delivered to the District. In addition:

1. The applicant agrees to pay a service charge for tapping the water main, with said service charge to be paid after receiving the estimate. You may also choose an outside contractor to perform this work but Pascoag Water Department will require staff to inspect it for compliance. The Inspection Fee is a minimum of one hour of labor and transportation costs.
2. The applicant agrees to pay for the materials and labor used to connect a new service from, and including, the curb stop to the place of service.
3. The applicant agrees to pay a one-time only, non-refundable meter use fee to Pascoag Utility District based on the size of the water meter. This fee covers the cost of any maintenance, repairs or replacement meters that may be necessary. However, unusual repairs necessitated by freezing, hot water, or attributable to any careless or neglect on the part of the owner, a fee will be charged to the owner of the property for a replacement meter.
4. The applicant agrees to pay a monthly meter maintenance fee to cover the basic cost of providing service, such as metering, meter reading and billing. These fixed costs are unaffected by the actual amount of water you use.
5. The applicant agrees to pay for all costs associated with the repair and/or replacement of water service lines and associated appurtenances (except the water meter), from, and including, the curb stop (shutoff near road), to the applicant’s end point of use. The only exception to this rule is if the residential applicant purchases Water Service Line Protection insurance.
6. The applicant agrees to pay any and all charges incurred for digging.
7. The applicant agrees to provide and pay for any necessary blasting.
8. The Pascoag Utility District will not perform or be responsible for grading or landscape work in connection with installing, maintaining or repairing water service connections.
9. The applicant hereby grants all rights of easement to the Pascoag Utility District in connection with installing, maintaining or repairing of water service connection, and agrees to execute any and all documents reasonably necessary to record or perfect such easement.
10. If at any time the water is shut off for nonpayment of water bills, or for any reason for which the Water Department is not responsible, there will be a service charge for restoring the service.
11. The applicant agrees to pay all costs associated with the purchase of water and any costs associated with exploration for new water sources.
